Before You Can Apply

Before you can apply for any of our Home Care Packages you will need an assessment from an Aged Care Assessment Team (ACAT). This assessment is needed to confirm your care needs and that you are eligible for a Home Care Package.

Call us on (02) 9832 5417 and our Customer Liaison Coordinator will take the time to answer all your questions and can help you to navigate the Aged Care System including obtaining the ACAT assessment.

Alternatively you can contact MyAgedCare on 1800 200 422 and ask for an ACAT assessment and a referral to Our Lady of Consolation’s Home Care Packages.

An ACAT assessment works out how well you're able to cope with daily living activities and your own care, as well as which services you may be eligible for.

An aged care assessment involves a visit from one or two members of the team, who will talk through the activities you find difficult to manage. They’ll then be able to discuss your options and let you know how to access the services that may be able to help you.

Once the ACAT has assessed your needs and have agreed that you are eligible, they can then make a referral to Our Lady of Consolation’s Home Care Packages

You can still contact your nearest ACAT and ask for an ACAT assessment directly. However, from 1 November 2015, ACATs will start using MyAgedCare to arrange assessments and make referrals, and from early 2016 onwards, ACATs will no longer accept direct referrals for assessments other than through MyAgedCare.
