Dear Staff Member,

URGENT ATTENTION: Renewal of Criminal History Check

In accordance with the Aged Care legislation it is mandatory for all staff, volunteers and contractors to have a current Criminal History Check certificate and that these checks have to be renewed every three (3) years.

Our records show that your Criminal History Check is now due for renewal in order for Our Lady of Consolation Aged Care & Services Ltd to comply with our obligations under the Aged Care legislation. OLOC will be paying for this check to be conducted electronically through fit2work.

Therefore, I am directing you to CLICK HERE to download the fit2work National Criminal History Record Check form to complete.

It is essential that when you completed the form and copied your “Proof of Identity” documents that you either email them to policecheck@oloc.com.au no later than 27 July 2020 or if you are unable to scan the documents please arrange for them to be delivered to the Reception in the Main Administration building by the due date.

Please note that failure to comply with this directive could lead to you being placed on unpaid leave until the Criminal History Check has been completed.

On the Police Check form please read Section 2: Proof of Identity carefully as the Proof of Identity requirements have changed. It is now necessary for you to verify your identity in line with Australia’s National Identity Security Strategy.

To achieve this, you must at a minimum:

  • Provide four (4) documents;
  • All four (4) documents cannot be drawn from a single category;
  • It is strongly recommended to try and use a document from each category.

In combination, your documents must include your full name, date of birth, and a photograph.
